Large tile is a flatness problem, not a tile problem

That is why levelling compound stops being optional under large format and natural stone, and why the subfloor line goes up under them. It is also why an uncoupling membrane earns its place: it lets the tile and the subfloor move at different rates rather than transmitting every seasonal movement into the grout.

Should the flooring go under the vanity and the toilet?

Under the vanity is a judgement call: running the floor underneath costs a little more in material and makes any future vanity change straightforward, while stopping at the cabinet saves material and locks you into that footprint. On a small floor the material saving is trivial, so running it through is usually the better call.

What is an uncoupling membrane and do I need one?

It is a thin layer that sits between the subfloor and the tile and lets the two move independently. Wood subfloors expand and contract with temperature and humidity; tile does not. Without something to decouple them, that movement goes into the grout and eventually the tile. In a high-desert climate with wide day-to-night swings, that is not a theoretical concern. It costs a few dollars a square foot on a small floor and it is the difference between a floor that lasts twenty years and one that starts cracking in three.

Is it cheaper to do the flooring with other bathroom work?

Yes, and by more than people expect. Protection, set-up and disposal cost roughly the same whether the room is 35 square feet or 110, so on a stand-alone floor they land entirely on that project. Done alongside a shower or tub replacement, they are already paid for — and the floor is being opened at the drain anyway, and the toilet often has to come off regardless. It is also the correct sequence: flooring goes in after the wet work and before the vanity and trim.
